We may be about to rediscover that dying is not such a badthing to do after all. Sir William Osier took this view; hedisapproved of people who spoke of the agony of death,maintaining that there was no such thing. In a 19th-century memoir about an expedition in Africa, there isa story about an explorer who was caught by a lion, crushed acrossthe chest in the animal's great jaws, and saved in the instant by alucky shot from a friend. Later, he remembered the episode in cleardetail. He was so amazed by the extraordinary sense of peace andcalm, and total painlessness, associated with his partial experienceof being killed, that he constructed a theory that all creatures areprovided with a protective physiologic mechanism, switched on atthe verge of death, carrying them through in a haze of tranquility. I have seen agony in death only once, in a patient with rabies,who remained acutely aware of every state in the process of hisown disintegration over a 24-hour period, right up to his finalmoment. It was as though, in the special neuropathology of rabies,the switch had been prevented from turning. We will be having new opportunities to learn more about thephysiology of death at first hand, from the increasing numbers ofcardiac patients who have been through the whole process and thenback again. ……
